I hereby agree to the processing of my personal data for the purpose of carrying out the request sent via the contact form as well as for the purpose of keeping records of processing activities.

Providing personal data is voluntary, but necessary in order to process the request and answer it by e-mail or telephone. 

Provided personal data will be processed on the basis of Article 6(1) letter a) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, hereinafter called the Regulation, that is on the basis of a consent which can be withdrawn at any time, by sending an appropriate message to the e-mail provided above. The withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal. 

The data subject has the following rights:
a) the right to request from the controller access to his personal data,
b) the right to request from the controller rectification, erasure or restriction of processing of his personal data,
c) the right to data portability (the right to receive from the controller personal data and transmit them to another controller)
pursuant to the provisions of the Regulation.

Where personal data are processed for direct marketing purposes, you have the right to object at any time to such processing of data. If you object to processing of personal data for direct marketing purposes, this data can be no longer processed for such purposes.

Personal data will be stored for the period of time essential for the purpose of processing and answering the request and for a period of up to 5 years since filing the request. 

The personal data recipients are the following categories of entities: e-mail service providers.

The data subject has the right to lodge a complaint with a supervisory authority, in particular in the Member State of his or her habitual residence, place of work or place of the alleged infringement if you consider that the processing of personal data relating to you infringes the Regulation.
